Your initial session(s) will involve an evaluation of your needs. By the end of the comprehensive psychiatric evaluation, I will be able to offer you some first impressions about indication for medication and any other treatment options. Should you decide to continue with my services, then we will work together to create a plan of care.
If you ever have questions about medication or plan-of-care, please feel free to discuss them whenever they arise. If your doubts persist, I will be happy to help you set up a meeting with another professional for a second opinion.

Create a Patient Portal Account
Select Patient Portal from any of the pages on this website and follow the prompts. Next, update your patient portal account to include your emergency contact and billing information. From the homepage please send our office a message with photos attached of the front and back of both your ID and insurance cards.

In order for me to manage my patient’s mental health needs appropriately, I require my patients to be available for their scheduled appointments.
I request that you provide me with at least 48-hours notice if you must change/cancel your appointment. Failure to do so will incur a fee as noted below.
This courtesy allows me to schedule another patient who is also in need of care.
No-Show/Late Cancelation Fees:
First occurrence is free of charge
$350 per hour; prorated based on length of scheduled appointment time
Three or more occurrences per calendar year may result in termination of provider-client relationship

For existing clients, I will check my emails every 24 hours and will respond to emails within 72 hours from the day they were sent, based upon my clinical judgement and the information provided.

For refill requests, please contact your pharmacy directly. If no refills remain on your prescription, your pharmacy will contact me to request a renewal of your prescription. Please allow 72 hours for processing refill requests.
Early refills of controlled substances will not be approved for any reason.
